Description
Care coordination in the U.S. is evolving to include social determinants of health. This enhanced coordination includes connecting patients to community resources such as transportation and housing as well as resources for elder and child maltreatment and food insecurity. There is little known about operationalizing these aspects of care coordination in rural settings. The authors' study was designed to determine care coordination practices in rural settings and to add to the limited literature on this practice. This study also contributes an in-depth reporting on the variety of service models being implemented.Learning Objectives

- Describe the components, challenges, and barriers of care coordination in general and in rural settings.
- Identify the essential components of care coordination.
- Discern how the organizations interviewed enhance rural care coordination through providing services and tracking process metrics.
