Description
Patients with sickle disease (SCD) are hospitalized about eight times per year and have high rates of mortality and morbidity. At the facility where this project took place, SCD was the one of the highest readmitting diagnoses with more than 46% of patients readamitted within 30 days of discharge. By using evidence-based care coordination interventions, this facility was able to significantly reduce the 30-day readmission rate for patients with SCD.Learning Objectives
