Description
The New York State Adult Home Settlement presents a variety of systemic, care management, and individual member challenges. Adding the Pathway Home approach to an already existing, yet insufficient care management model strengthens the initiative?s goal to transition and retain members safely into the community. Class member self-efficacy and cross-system collaboration are essential in facilitating a class member?s move into the community. Current low-touch care management programs are insufficient for members with complex needs living in institutions. These care management programs need to be augmented with a whole person approach, delivered by a multidisciplinary team.Learning Objectives
After completing this continuing education activity you will be able to:
- Identify the services and processes related to transitioning adults with SMI from NYS-licensed adult homes into alternative community housing with wrap-around services.
- Explain the Adult Home Plus (AH+) program and the Pathway Home (PH) approach.
- Describe the authors' findings about the use of the Pathway Home Adult Home Plus (PH AH+) model and case management implications.
Learning Outcomes
By the conclusion of this activity, a majority of the participants will demonstrate knowledge of implementing the Pathway Home program to assist adults with serious mental illness (SMI) living in a New York State (NYS) licensed adult home in transitioning into alternative community housing by achieving a passing score on the posttest.Disclosures
