Description
Hospital readmissions occur for many reasons with deleterious effects on patients and financial costs to the health care system. Most 30-day readmissions occur among Medicare beneficiaries. The authors describe a QI project that piloted ICM to reduce 30-day hospital readmissions among this cohort. ICM was provided via an academic?clinical partnership at no additional costs to the health care system.
