Description

Hospital readmissions occur for many reasons with deleterious effects on patients and financial costs to the health care system. Most 30-day readmissions occur among Medicare beneficiaries. The authors describe a QI project that piloted ICM to reduce 30-day hospital readmissions among this cohort. ICM was provided via an academic?clinical partnership at no additional costs to the health care system. Those who received 30 days of ICM, compared with those who did not, stayed out of the hospital longer These results speak to using academic? clinical partnerships to provide a sustainable way to reduce readmissions among medically and socially complex populations.
