Description
Community-acquired pneumonia, the most common cause of infectious death globally occurs frequently and with high morbidity and costs along with high mortality. Quality improvement initiatives for hospitals and skilled nursing facilities (SNFs) include decreasing unplanned inpatient pneumonia readmissions within 30 days of discharge from the index hospitalization. This study focused on an evidence-based pneumonia care bundle directed at patients who are at high risk for a hospital readmission from a skilled nursing facility (SNF). The authors? concluded that this pneumonia care bundle is affordable, not complicated to use, and easy to replicate.Learning Objectives
