This integrated literature review evaluated the published studies on how postacute care collaboration ensures a continuum of care and reduces heart failure (HF) readmissions. The authors findings included that multidisciplinary management of HF patients decreased HF 30-day readmissions. As the case management role moves into outpatient care, better there are opportunities to coordinate services across settings.Learning Objectives
