The authors consider how well positioned professional case managers are to address SDOH through care coordination. Care coordination is integral to case management practice with new opportunities being identified on an ongoing basis. As long as patient care continues to move outside the hospital, care coordination must also. The authors conclude that case managers are leading the charge to address both the physical and behavioral health needs of their clients.
