Both articles must be read before accessing test. In this two part series, the authors set the stage for the 2019 Centers for Medicare & Medicaid (CMS) changes to Medicare Advantage plans and the potential impact on case management practices. These changes allow additional benefits for nonmedical home services. The effects will be experienced in long-term services and support. The article outlines the role of case managers in supporting clients to remain in the community setting safely by managing transitions. the authors explain how LTSS is rated, regulated and accredited. They include discussions of quality measurements and outcomes. Through case study examples, the authors highlight the key role case managers have as advocates for their clients, helping them to improve their health and well-being. To achieve these ends, case managers must have an understanding of the population demographics, terminology, available programs and qualification criteria for LTSS.
