The author provides information about the development of the Kilgore Heart Failure Case Management (KHFCM) Acuity Tool. The primary objective was to design an acuity tool that could measure heart failure case manager workload. The implementation of this tool in a health maintenance organization (HMO) resulted in several quality improvement outcomes.
