Description

The Early Screen for Discharge Planning (ESDP) originated in an urban academic medication center. The authors hypothesized that the use of the ESDP would differentiate between who would and would not benefit from early intervention by discharge planners in small, rural hospitals. Their findings both supported this hypothesis and also the integration of EDSP in the discharge process in regional, community hospitals.
